All student information previously collected by Salem Keizer School District for student Athletic Participation will need to be entered into the FamilyID system.  If you have a current sports physical on file at the school, this information will be added to the online registration for you. If you do not have a current physical on file at school, you will need to visit a doctor and attach the completed physical form to your online registration or bring it into the main office.  Please see the links section to print out a copy to take to the doctor.

REGISTRATION PROCESS:  Parent/Guardian should register click on link:

  • To find your program, click on the link above and click on “Find a Program”
  • Type in the name of your Middle School and Select State – Click Find
  • Under Featured Organization:  Click – Houck Middle School
  • In the upper right “click” on Create Account
  • If this is your first time using FamilyID, click Create Account.  Click Log In, if you already have a FamilyID account.
  • Create your secure FamilyID account by entering the account owner First and Last names (parent/guardian), E-mail address and password. Select I Agree to the FamilyID Terms of Service. Click Create Account.
  • You will receive an email with a link to activate your new account. (If you do not see the email, check your E-mail filters (spam, junk, etc.).
  • Click on the link in your activation E-mail, which will log you in to
  • Click on blue link for sports program.
  • Once in the registration form, scroll down to “register now” (green button) “click” complete the information requested. All fields with a red* are required to have an answer.
  • Click the Continuebutton when your form is complete.
  • Review your registration summary.
  • Payment Options and Alternate Payment Method (check or cash to the Houck’s Main Office Or online payment through Salem Keizer School District
  • Click the green Submitbutton. After selecting ‘Submit’, the registration will be complete. You will receive a completion email from FamilyID confirming your registration.`

At any time, you may login at to update your information and to check your registration(s).

To view a completed registration, select the ‘Registration’ tab on the blue bar.
